Risk factors for recurrence of Crohn’s disease requiring surgery in patients receiving post-operative anti-tumor necrosis factor maintenance therapy

چکیده
Objectives: Anti-tumor necrosis factor (TNF) antibodies have shown efficacy in the prevention of recurrence of Crohn’s disease after intestinal resection. However, some patients develop surgical recurrence despite this therapy. We aimed to evaluate the risk factors for recurrence of Crohn’s disease requiring surgery while receiving post-operative anti-TNF therapy. Methods: We performed a retrospective evaluation of 164 patients who had received post-operative anti-TNF maintenance therapy between 2002 and 2016. We classified Crohn’s disease-related re-operation as surgical recurrence and analyzed its risk factors using the Cox proportional hazard model. Results: Of the 164 participants, 128 had received infliximab and 36 had received adalimumab maintenance therapy. We obtained follow-up data over a mean of 60.2 months. The proportion of patients with surgical recurrence at 5 years was 14.9%. The only independent risk factor for surgical recurrence, which we identified was post-operative smoking habit (odds ratio, 5.03; 95% CI, 1.1412.8; P=0.033). Conclusions: Post-operative smoking may be a significant risk factor for post-operative surgical recurrence of Crohn’s disease while receiving anti-TNF maintenance therapy.

Optimizing post-operative Crohn’s disease treatment
Despite the availability of biological drugs and the widespread and earlier use of immunosuppressants, intestinal resection remains necessary in almost half of the patients with Crohn's disease. The development of new mucosal lesions in previously unaffected intestinal segments (a phenomenon known as post-operative recurrence, POR) occur within the first year in up to 80% if no preventive measu...
متن کاملPrimary Cutaneous Nocardiosis in a Patient Taking Adalimumab Therapy for Crohn’s Disease
INTRODUCTION The introduction of biologic immunomodulators, in particular antibodies targeted against tumor necrosis factor alpha (TNF-alpha), has revolutionized treatment of Crohn’s disease. However, this comes at the expense of a higher risk for opportunistic infections due to a generalized immunosuppressive effect. Bacterial and opportunistic infections are well-known complications of anti-T...
متن کاملIleal Crohn's disease is best treated by surgery.
The optimal management of Crohn’s disease remains complicated and controversial. In common with other benign conditions, treatment is dependent on a balance between therapeutic risk and benefit. Unfortunately, in surgery the risk arm of the risk/benefit balance appears to predominate. Clinicians, both surgical and medical, seem preoccupied by the complications of surgical intervention, includin...
متن کاملP-211: Recurrence Rate of Ovarian Endometriomas after A Laparoscopic Excision
Background: Endometrioma is a common cause of gynecologic morbidity and the most common benign ovarian cysts are found at surgery. One of the most troubling aspects of treating endometrioma with laparoscopic excision is disease recurrence after surgery. Present study was conducted to evaluate risk factors for the recurrence of endometrioma after first laparoscopic excision. Materials and Method...
متن کاملIncreased risk of post-operative complications in patients with Crohn's disease treated with anti-tumour necrosis factor α agents - a systematic review.
INTRODUCTION Tumour necrosis factor α (TNF-α) plays a role in the immune defence, angiogenesis and collagen synthesis. Inhibition of these pathways may increase the risk of infections and impair wound healing in patients after surgery. Biologic treatments including anti-TNF-α agents are increasingly used in the treatment of inflammatory bowel disease. Taking into consideration the biologics' me...
